Sage Advocacy’s mission is to promote, protect and safeguard the rights and dignity of older people, vulnerable adults, and healthcare patients through individual and systemic advocacy. Sage provided information, support and advocacy services to more than 12,000 people (cases & queries) in 2024 and demand for its services is growing. The work of Sage on behalf of clients is independent of family, service provider or systems interests. The service is free of charge and confidential. In the context of recent and planned legislative, health and social care developments, of growing demand for our services and of our 2nd Statement of Strategy ‘The Next Stage’, Sage is now recruiting for the following roles:

Regional Advocate Focused on Care Transitions – Dublin x 2

The purpose of the Regional Advocate Focused on Care Transition is to provide an enhanced independent advocacy service to older people and vulnerable adults in transitions between care settings with a particular emphasis on supporting hospital and community-based staff in managing transfers of care and transitional care issues related but not limited to decision- making capacity, safeguarding, protection of liberty, end of life care and identifying systems issues that arise. The work will involve building effective relationships with senior management teams of integrated care areas and the acute hospitals within them. Regional Advocate Recruitment pack.

Regional Advocate Casework and Court Reports Southeast x 1

The purpose of the Regional Advocate is to lead on complex advocacy cases supported by case management safeguarding legal and data analysis skills. Secondary purposes include: provision of support and advocacy services to key groups and individuals such as institutional abuse and vulnerable older persons with little or no circle of support; growing awareness of Sage advocacy and its services at regional level; developing a deep understanding of an strong working relationships with the structures and systems of the emerging HSE Regional Health Authorities; ‘mapping’ service providers and resources based on national guidelines; identifying and developing opportunities to tackle systemic issues at regional and national levels. Service Support Coordinator x 1

The Service Support Coordinator will provide high-level administrative and operational support to the Case Management and Support (Asst. CEO) and the wider service team, which includes the Management Team, Regional Managers, Information & Support Services Manager. This role

is critical in ensuring the smooth running of service-related activities, accurate data management, and timely reporting across internal and external stakeholders. Service Support Recruitment pack.
