Job Title: Case Management Worker - Homeless Health Peer Advocacy Team
Depaul is seeking people who are passionate about helping vulnerable and marginalised people to joinThe Homeless Health Peer Advocacy Team. Collaborating with the inclusion health service team in the Mercy Hospital, the Case Management Worker will assist service users access healthcare and make appointments.
Key Areas of Responsibility
- Engage and coordinate support services across community, hospital, and accommodation sectors for Inclusion Health clients facing homelessness who face complex health, addiction, and/or mental health challenges.
- Provide assertive support to clients from diverse backgrounds and manage relationships with external health and service agencies, including hospitals, GPs, researchers, and city council.
- Assist medical professionals and peer advocates in crisis management and ongoing care coordination, and support clients post-discharge to ensure their care plans are followed.
- Work directly with service users using a case management approach to access necessary services, advocate for clients, and support tenancy sustainment.
- Maintain effective interagency relationships, adhere to data protection and confidentiality policies, participate in relevant meetings, and contribute to service improvement projects and data collection.
The above list is not exhaustive; additional areas of responsibility may be added over time and flexibility to cover for other staff roles is required from time to time.
Person Requirements
- Min. 2 years’ experience working in the homeless sector or related field. One year coordinating support plans for service users with complex needs.
- Understanding of how and why people become (or are at risk of becoming) homeless.
- A clear understanding of the case management approach.
- Experience of interagency working alongside various statutory and voluntary services that provide health supports and wraparound services to those with complex needs.
- Be an excellent communicator and advocate who is solutions focused, person-centred, has acreative approach, who is experienced in lone working environments.
- Lived experience of homelessness will be taken into special consideration.
